State law and the E.O. Smith Board of Education require that the administration of any medication in school must have the proper medical paperwork completed and on file in the health office. The medical form must be completed and signed by a medical doctor with proper dosage and administration instructions. The form must also be completed and signed by parent. All medications must be brought into the school health office and given to the nurse by parent/guardian. Medications may not be brought to school by the student. Medications must be in the original pharmacy labeled container.
Inhalers may be self-administered and self-carried providing the necessary forms are completed and on file in the health office.
Students are not permitted to carry any medication in school. This includes Tylenol, Advil or any other over-the-counter medications.
Forms are filled out on Infosnap each year to allow the nursing staff to administer Tylenol or Ibuprofen. These forms must be completed and on file in the health office to allow the nursing staff to administer these medications to students.
