What Do I Need for Sports Clearance?
- Health History: Completed, signed and dated annually (after August 1st) by parent and student. Annually=school year=August 1st to June 30th.
- Physical (NEW UPDATE-10/2020): Completed, signed and dated annually by physician. Valid for thirteen months from date of exam. Exam must cover the entire athletic season.
- Impact Testing: Baseline tests are completed twice. Once as a freshman and again as a junior. All parents will receive notification either via email or a letter in June informing them of the test dates and how to register for a time slot. New athletes (regardless of grade), who have never completed a baseline test need to register for a time slot.
- All paperwork (physical forms by doctor and health history by parent and student) are to be delivered to the health office.
- Health office staff will review paperwork and determine if above criteria have been met.
- If criteria have been met....Athlete/student receives from health office a form known as "sports clearance".
- Athlete/student hands in "sports clearance" form to their respective coach in order to participate in tryouts.
- Athletic clearance must be obtained PRIOR to tryouts to be able to participate.
- Clearance is done during regular school hours once the school year is in session.
- An email or a letter is sent home to the entire student body in June providing the clearance dates and times for those athletes participating in fall sports in which the tryouts take place prior to the start of the school year.
